About the Role The Nurse Practitioner (NP) will provide and coordinate comprehensive care using advanced practice skills in the management of ambulatory cancer patients and those referred to the Cancer Rapid Assessment Unit. The role of the Nurse Practitioner in the Division of Cancer Services will: Establish and operationalise a model of care for this new service. Function in a collaborative model with the healthcare team to provide assessment, diagnosis, treatment and referral for ambulatory cancer patients. Provide safe, quality, cost effective care and practice within the Nurse Practitioner's approved Practice Scope that is consistent with contemporary standards and evidence-based practice. Work collaboratively with the health care team and contribute to enhanced patient outcomes by actively leading and supporting existing and emerging nurse-led models of care whilst maintaining flexibility and responsiveness to evolving patient and service needs. Provide referrals to appropriate agencies, including primary health settings. Provide coordinated patient centred care, creating partnerships, improving patient outcomes and facilitating systems improvement.
