You will be accountable for:
- Facilitating smooth business operations between the GPs we support and residential aged care facilities (RACFs) including:
- education of GPs and RACFs on our processes
- onboarding of GPs and RACFs
- facilitating effective communication and problem-solving
- Clinical governance and management
- Maintaining our clinical database to Accreditation standards
- Designing and implementing a learning and development plan for:
- the nursing team members to foster continuous improvement
- quality improvement projects as per PIP QI requirements

Aged Care GP supports doctors working in various Residential Aged Care Facilities across Greater Melbourne. We are an innovative, friendly team with a strong drive to streamline how doctors work in Aged Care.
We provide targeted support to GPs to improve their working conditions and promote long term relationships at RACFs. This improves resident quality of life outcomes for this vulnerable population.
