About the role:
You will provide services to participants and families through assessment and provision of support services and assistance in the development and evaluation of support and intervention programs. Reporting to the CEO, your accountabilities will include:
- Therapeutic use of occupations, and activities, including therapeutic use of self (including one’s personality, insights, perceptions, and judgments, as part of the therapeutic process)
- Report writing for NDIS, Medicare, Veterans Affairs, Workcover QLD and other relevant reports
- Skill development in self-care, self-management, home management, and community/work/school reintegration
- Education and support of individuals, including family members, caregivers, and others, through collaborative and consultative partnerships and family-centred approaches
- Care coordination, case management, transition services including discharge planning, and onward referral to relevant services
- Assessment, customisation and oversight of a client’s functionality
- Use of natural contexts for assessment and intervention (i.e. home, school classrooms, work settings, community)
About You:
- a relevant tertiary degree, with full registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Previous experience within NDIS or healthcare client supports desirable.
- Lived experience within the disability and / or community services sector highly advantageous.
- Experience with Microsoft Office Programs such as Excel, Outlook, PowerPoint, Word and Client Management systems such Halaxy would be advantageous.
- An ability to establish rapport, build and maintain trusted relationships with clients
- Experience leading and developing a high-performing multidisciplinary team environment.
- Current drivers license and access to own vehicle when visiting clients
