Join our Memory Support Transition Service team as a Occupational Therapist and help support our commitment to quality patient care.
The Occupational Therapist participates as a member of the Memory Support Transition Service team, which will provide support to patients with memory loss and Behavioural and Psychological Symptom of Dementia (BPSD) with the aim of safely transitioning them from the Acute and Subacute inpatient environment into community accommodation. The role will work closely with the Geriatric Evaluation and Management (GEM) Nurse Practitioner Candidate and the Multi-D team to develop and action patient centred management plans to support patient well-being and transition to new environments.
About the Role:
As the Occupational Therapist, your duties may include:
- Assessing patients’ occupational abilities, problems and needs using a variety of formal and informal assessments
- Planning and implementing intervention programs
- Working closely with Residential Care Facility staff to support the integration of care between SALHN and the community setting
- Educating and supporting Residential Care Facility staff to care for patients with memory loss and BPSD in line with the patient's documented behaviour support plan
- Case management, care planning and liaison with older patients in the community.
To be successful in this role, you will need:
- A degree or other qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia
- Experience as a clinician in the assessment and provision of high-quality Occupational Therapy services to clients with a variety of conditions requiring management of issues associated with cognitive impairment and/or changed behaviours
- High level interpersonal, negotiating, counselling and problem-solving skills with the ability to communicate with patients, other professional staff and community agencies both orally and in writing.
- Ability to critically analyse or evaluate issues related to Occupational Therapy care in a manner which can be applied to quality activities, service evaluation, professional education or research with a commitment to ongoing professional development.
