The role:
Our client is seeking a dedicated professional to join their dynamic team, coordinating multiple services across various sectors to ensure collaborative and integrated support for those in need. In this position you will support participants over eighteen availing predominantly of NDIS funding and also other funding streams.
In this role, you will be responsible for conducting comprehensive assessments, developing action plans tailored to clients' individual needs, and delivering recovery-oriented interventions. You will work closely with clients, families, carers, Support Coordinators, and other providers to deliver high-quality, responsive services that aim to minimize preventable hospital admissions and enhance overall health and wellbeing outcomes.
You will facilitate clients’ access to our client's programs and services, undertake initial assessments, and coordinate direct client services. Your ability to advocate for clients, provide specialized education, and utilize a client-centered approach within a multidisciplinary team will be key to achieving effective health outcomes. Additionally, you will be involved in developing group programs, liaising with various health and social service providers, and supporting clients in accessing appropriate services.
Compliance with relevant legislation and organizational policies is crucial in this position. You will be expected to uphold professional standards, engage in continuous professional development, and contribute to special projects as required. Essential Criteria
- Minimum Bachelor degree in Occupational Therapy or relevant discipline
- AHPRA registration
- Valid NDIS Check
- Police Check
- Working with Children Check
