Your Role:
- Conduct standardised clinical assessments to determine goals and areas of needs
- Develop functional, routine-based goals with clients and families
- Review progress in collaboration with individuals and families to celebrate achievement and set new goals
- Work closely with key stakeholders providing information, education and support
- Ongoing evaluation, training and review to ensure the delivery of quality services to clients
- Provide evidence informed therapy solutions for your clients
- Deliver a mix of capacity building, coaching, 1:1, small group therapy, consultation and truly MDT wrap around supports
- Provide services in a variety of locations – clinic, home visits, community centres, schools, even under a tree in the local park!
What we need from you:
- Tertiary qualifications in OT or SP
- Exceptional communication skills that enable you to work in a culturally rich and diverse environment and collaborate in a multidisciplinary team
- Experience working with NDIS, private, Medicare, Care Plan, Home Care Package and/or Residential Aged Care is always a benefit.
- Compliance – AHPRA Registration, Working with Children Check, National Police Check, NDIS workers screening check, Covid-19 Vaccination Certificate, First Aid Training, Driver’s License & Vehicle
