We are currently seeking a dedicated and experienced Palliative Care Program Manager to join our team. As a Program Manager, you will be responsible for Case Managing and delivering high-quality care services to clients receiving Palliative care/EOL care. You will work closely with clients, their families, and a multidisciplinary team to assess needs, develop care plans, and ensure the provision of personalized care that enhances clients' well-being and independence.
You will love this opportunity to provide vital support to our Clients, as you:
- Provide high-quality case management
- Organise, coordinate and monitor clients' support services;
- Connect clients to services in the community to ensure their needs are fully met;
- Ensure effective and financially accountable service provision based on specified and desired goals and outcomes;
- No supervisory responsibilities;
- Be flexible and respectful in managing clients and responsive to client needs and changes in circumstances; and
- Collect client data and maintain electronic records and paper files.
- The ability to work independently and monitor individual workload, juggle multiple concurrent tasks and meet deadlines.
- A working with vulnerable people (WWVP) card or be able to successfully register for one
- A current drivers license
