  This is a Recruitment Pool for Fixed Term Full Time, Fixed Term Part Time at various hours until 30/06/2025, with the possibility of extension(s).

Suitable applicants will be placed in a pool from which appointments may be made when similar vacancies occur. Appointments may be made from this pool until 30/06/2025.

Position Profile: We are seeking to appoint a suitably experienced and motivated individual to the Patient Journey Navigator (Community Outreach Officer) role within the Country PatHS Program team. The successful applicant will engage with, and support vulnerable country people and their support persons travelling to Perth to access health care. This role will develop and maintain relationships with country patients and their support persons, and work collaboratively with a number of agencies and promote engagement with the health system with a strong focus on providing support for a culturally safe stay and delivering patient centred care.
