About the role

The Saint James Transitional Care Unit (TCU) residential service, delivered in partnership with East Metro Health Service network, supports clients aged between 18-64 years up to 6 months to transition from a hospital setting back into the community or to prevent admissions whilst residing in the community. TCU offers recovery-focused, person-centred supports delivered by specialised mental health clinicians and peer workers a shift from treating symptoms to coaching people to develop a life with meaning and purpose. Clients are supported to achieve their recovery goals, build life skills, develop links with support networks, strengthen relationships with families and carers, participate in work or education, and grow capacity to live independently.

We are seeking a compassionate, resilient Peer Practitioner for a fixed-term, part-time (19.5 hours per week) opportunity to work alongside a caring team in the St. James area. You will utilise your lived experience of mental ill health and recovery to provide empathetic, person-centred supports to clients to achieve their wellbeing goals, build capacity and daily living skills, strengthen social connections and community networks, and live independently in the community.

Key responsibilities

  • Utilise your own lived experience of mental ill health and recovery to inform your work and that of the team.
  • Support clients return to their community and enjoy a meaningful life through an individual approach to recovery.
  • Deliver high quality services and facilitate positive educational and social experiences.
  • Plan and deliver group programs that aid clients to build their skills and focus on recovery.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to the client’s recovery goals.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Prior lived experience and willingness to support clients through sharing learnings and recovery orientated practice is required.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
