Position: Pharmacist – Medication Management

We are seeking a motivated and compassionate Pharmacist to join our dynamic team.
Position Overview:
As a Pharmacist at Claims Pharmacy, you will play a crucial role in overseeing and implementing Medication Management programs designed to support individuals at risk of medication dependence, addiction, and overdose. You will work closely with our multidisciplinary team to develop and execute strategies that prioritise the well-being of our clients.
Key Responsibilities:
Client Assessment and Planning:
- Conduct thorough assessments of clients to understand their medication history, current health status, and potential risks.
- Collaborate with healthcare professionals to create personalized Medication Management plans.
Program Implementation:
- Develop and implement Medication Management programs that address individual needs and mitigate the risk of addiction or overdose.
- Monitor and adjust programs as necessary based on the client's progress and evolving health conditions.
Client Education:
- Provide education to clients on the safe and responsible use of prescription medication.
- Offer guidance on potential risks and side effects, as well as alternative strategies for pain management or health improvement.
Team Collaboration:
- Work closely with pharmacists, physicians, counsellors, and other healthcare professionals to ensure a holistic and coordinated approach to Medication Management.
- Participate in regular team meetings to discuss client cases, share insights, and optimize program strategies.
Documentation and Reporting:
- Maintain accurate and up-to-date client records in compliance with legal and ethical standards.
- Generate regular reports on program effectiveness and client progress.
Qualifications:
- Bachelor of Pharmacy or equivalent with current Pharmacist Registrationwith AHPRA.
- 2yrs+ general registration is preferred.
- Excellent communication and interpersonal skills.
- Strong organizational and problem-solving abilities.
- Ability to work collaboratively in a multidisciplinary team.
