Your opportunity:
- The GEDI (Geriatric Emergency Department Intervention) team provide a frontload service with a multidisciplinary approach to the screening, assessment, care coordination and care planning of the frail elderly (generally aged over 70 years) who present to the Emergency Department. The GEDI team work collaboratively with the older person, their caregivers, and the broader healthcare team to help coordinate a mutually acceptable patient centred plan in the Emergency Department context. GEDI engage in a range of activities to ensure appropriate clinical and disposition decisions are made in consultation and recommendations with the treating Emergency Department physician, GPs, allied health practitioners, nursing and the acute Geriatric service. The GEDI team minimise post-discharge gaps that can result in Emergency Department return visits or hospitalisation.
- The purpose of this position is to deliver generic geriatric assessments, case management & routine physiotherapy clinical services as part of the GEDI team, in accordance with professional scope of practice, competencies and ethical standards.
Prior to being made an offer of employment, you will be required to demonstrate immunity to the following Vaccine Preventable Diseases (VPD): Hepatitis B, Measles, Mumps, Rubella, Varicella (chicken pox) and Pertussis (whooping cough).

You will receive a competitive remuneration package value up to $128,706 p.a., comprising salary between $76,747 - $112,805 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Permanent Full Time, 76 hours p.f.)
