We are a busy family-owned General Practice based on the Sunshine Coast looking for an experienced Practice Nurse to join our team. We are committed to enhancing the health and well-being of our patients. We offer a supportive team-based environment in which the Practice Nurse will work closely with GPs to deliver high-quality, individualised care.
Position Profile:
Provide high quality, supportive primary health care services to the community and be part of a larger multi-disciplinary team with a focus on improving health outcomes for our patients.
Practice Nurses contribute a valued and complementary role to the General Practitioners and to our patients. They provide a variety of services that enhance the quality of patient care in line with RACGP standards and quality improvement measures.
Key Criteria:
- Registered Nurse licensed to Practice (AHPRA)
- Previous experience working in a General Practice
- Experience with Best Practice software and Hot Doc essential
- Experience working with a diverse range of people
- Excellent knowledge of chronic disease and care pathways
- Understanding of the Medicare Benefit Schedule – especially in relation to Chronic Disease and Practice Nurse item numbers
- Experience with heart health assessment, TCA’s, CVC’s and GPMP’s
- Experience with the National Immunisation Program and COVID vaccines
- Capacity to multitask, prioritise and good time management
- Must be pro-active with a positive attitude
- Able to work as part of a team
- Excellent communication and time management skills
- Professional presentation
Job Requirements:
The Practice Nurse will:
- Assist in the delivery of medical services, patient triage, acute presentation support, minor procedures, wound care and immunisations.
- Assist in the delivery of the COVID-19 vaccination program.
- Support patients with Chronic Disease management, including working closely with the General Practitioner around the preparation of management plans, referrals to allied health services and referrals to health programs.
- Regularly review and maintain the recall and reminder register.
- Undertake health screening and health monitoring activities including ECG and spirometry, as well as other clinical tasks requested by the General Practitioner.
- Co-ordinate and implement preventative health activities including health assessments and education.
- Implement and monitor infection control processes, including compliance with sterilisation guidelines.
- Maintain the cold chain systems and other medicine storage systems.
- Co-ordinate the treatment room including stock control and general maintenance and care of medical instruments and equipment.
- Participate in the accreditation process.
- Assist with Reception duties.
