Qualifications & experience
- AHPRA Registered Nurse
- Experience in Chronic Disease Management
- Strong organisational and time management skills
- General practice nursing experience
- Manage CDM/HA and CVC patients
- Wound Care, ECGs, Assist doctors in procedures
- Patient Recalls and Reminders
- Ability to triage, monitor and maintain cold chain, Infection control, sterilise instruments and stock control
- Adult/Child & Baby Immunisation
Tasks & responsibilities
- Lead Chronic Disease Management within our centre, Care Plans, Team Care Arrangements and Health Assessments
- Empower patients to take ownership for own health
- Work with GP's to promote preventative healthcare throughout the practice
- Increase the number of patients who are eligible to receive a care plan for the first time
- Manage administration tasks relating to CDM/HA appointments
- Use systems daily, weekly and monthly to extract data
- Triaging patients, over the phone and in person
- General day to day work within treatment room
- Sterilisation of instruments
- Immunisations / Vaccinations
- Assist in reception if needed
- Ear Syringing
