SalaryWe are a small family orientated practice looking for a great Practice Nurse with an interest in chronic disease management to join our team.

Tasks and Responsibilities
As well as basic nursing duties, the position is predominantly driven by Chronic Disease Management principles.
- Working with patients to complete care plans and prepare relevant allied health referrals is required.
- Providing health education to patients on various topics, including disease prevention, medication management, and lifestyle modifications.
- Undertake ECG's and fit 24hr holter monitors.
- Understanding of basic wound care techniques and suture/staple removal would also be advantageous.
- Skills in completing spirometry would also be valued.
- An interest in completing annual Health Assessments according to patient age and need is necessary.
- A willingness to work as part of a team with our patient's goals in sight is mandatory, and foresight to engage them in their healthcare by making ongoing appointments as required to promote optimum health.
- Knowledge of vaccinations and the National Immunisation Program would be beneficial, as would administration of IM and S/C injections.
- Maintaining accurate medical records and ensuring compliance with regulatory standards.
- Familiarity with Best Practice software would be advantageous, or the ability to learn on the job.
