The Housing Mental Health Pathways Program aims to reduce the risk of homelessness for people with mental illness following discharge from the Adult Acute Unit (AAU), Ballarat Mental Health Services, and reducing the likelihood of people re-presenting for Mental Health inpatient care by:
- Acting to maintain existing accommodation that has been placed at risk due to admission to hospital.
- Undertake assessment and advocacy work for people during their hospitalisation at the acute Mental Health unit.
- Arrange appropriate emergency/transitional accommodation for the client on discharge from the AAU.
- Provide Case Management support to the client for between three to six months after discharge from hospital.
- Support clients to find and establish a tenancy and sustainable housing, with the aim to empower individuals to maintain their own lease, develop local networks, and live as independently as possible.
If you have the below skills & experience - we would love to hear from you!
- Diploma/Advanced Diploma/Associate Degree, Community Studies (Welfare) Social Work or equivalent, or other relevant qualification, which may include mental health and wellbeing, with a focus on homelessness services.
- Demonstrated experience in service coordination and the application of case management principles and practice, including experience and understanding of risk and needs assessment frameworks and applications.
- Demonstrated experience in working with adults who are or at risk of homelessness and a demonstrated understanding of accessing housing services.
