Your Opportunity
The Housing Accommodation Support Initiative (HASI) is an adult mental health recovery program which provides complex case management and direct support in the community for individuals who are at risk of repeated hospitalisation and/or homelessness. The HASI program aims to reduce hospitalisations, homelessness, and mental distress, by increasing independence, building resilience and encouraging mental health recovery for adults with severe mental illness
Your key responsibilities will to:
- Provide psychosocial support to consumers clinically assessed as having severe mental illness, with high levels of psychosocial impairment to increase skill, knowledge and independence regarding activities of daily living such as tenancy maintenance,
- Assist consumers to connect and access community based social activities,
- Provide psycho-education and support to manage mental distress regarding experiences of mental health,
- Coordinate and attend appointments with consumers to support psychosocial recovery
- Assisting consumers in the creation, implementation, and review of holistic person centred support plans through a range of activities,
- Provide comprehensive community care coordination and advocacy,
- Develop and maintain relationships with internal and external stakeholders including service partners, government agencies, and external supports
- Assist in the promotion and awareness of HASI and its involvement in community activities
- Assist consumers to improve their quality of life and to enhance their skills to continue living independently in the community.
Requirements for success
- A relevant tertiary qualification with minimum of Cert IV in mental health
- Relevant work experience in mental health or community services environment
- Understanding and knowledge of the recovery orientated practices
- Strong communication and relationship building skills
- A valid NSW driver's license with high level confidence driving in a wide catchment area
- Must be able to work weekends and evenings
Before starting work with us, you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people.
