The Role
Interchange is seeking a knowledgeable, caring and pro-active employee to join our Regional Assessment Service team. We will be offering fixed term contract positions, in line with Regional Assessment Service government funding.
We have a position available within our My Aged Care Regional Assessment Service team in Wollongong and surrounding regions. This role will be 3 days per week, Tuesday, Wednesday and Thursday.
Travel is an expectation in this role, which includes driving to the Illawarra and Shoalhaven areas, and regional areas as required.
You will be reporting to the RAS Team Leader and must also be able to work autonomously.
The role responsibilities include:
- Completing face-to-face assessments to determine older Australians’ eligibility for support within My Aged Care and the Commonwealth Home Support Programme (CHSP).
- Reviewing previously assessed clients.
- Undertaking client-centred support planning using a wellness and reablement approach.
- Liaising with referral sources and community support organisations to assist our clients to obtain the home supports they require.
- Ensuring accurate and timely completion of assessments and documentation, utilising the My Aged Care online portal.
- KPI based.
Preferred candidates will have
- Completed My Aged Care, Home Support Assessor Training (SOA2) or willingness to obtain.
- Experience in a similar role, preferably Aged Care and/or allied health background.
- Ability to work independently, and as part of a team.
- High level of computer literacy skills
- Ability to follow guidelines relevant to the position as well as demonstrated problem-solving capacity.
- Current Australian driver’s licence
- Valid National Criminal History Check and Working with Children Check (NSW or equivalent).
- Current First Aid certificate
- Up to date COVID vaccinations
