The Community Connections Program supports people to increase their independence in the home and to build stronger social and community connections. Strong social connections can improve quality of life and provide better health outcomes.
The Regional Coordinating Partner will coordinate the program across their region, ensuring that all other Community Partners work together to provide the most appropriate services and support for participants.
The position of Regional Coordinating Partner comprises of 30% direct case work and 70% community work, data collection, reviews and meetings.
The Regional Coordinating Partner will be required to:
· Provide service coordination to the Community Connections Program within their region.
· Work individually with clients to support their inclusion into community programs using a case management framework.
· Identify gaps in services and develop strategies to ensure community partners can meet community support needs.
· Engage and partner with community members, stakeholders and other organisations to determine appropriate service options for clients.
· Develop strong knowledge of, and professional relationships with the various service areas across the region.
· Participate in regional coordinating networking opportunities to maximise collaboration.
· Provide coaching and mentoring to develop Coordinating Partners skills and competency to ensure the effective delivery of client support.
· Collect and maintain data and records in accordance with program requirements.
· Provide written monthly reports in accordance with program requirements.
This is a full time fixed term contract until 30 June 2025, based in Port Augusta, South Australia.

About You
The ideal candidate will have:
· A formal qualification in a Human Services or Management related discipline.
· Experience in providing case management, assessment and referrals.
· Experience in working with people from Culturally and Linguistically Diverse (CALD) and Indigenous backgrounds.
· Highly effective negotiation and communication skills.
· Experience in file management and reporting on accurate data and information.
· Experience in working in a highly confidential manner.
· Demonstrated ability to work independently and as a cooperative member of a team to achieve team objectives.
· Ability to apply initiative, critical thinking and problem-solving skills.
· High level of ability in using Information Technology and Microsoft Office programs.
Requirements of the role:
· Hold (or willingness to obtain) a DHS Working With Children Check – don't worry, we can support this process if necessary!
· Hold (or willingness to obtain) a current National Criminal History Record Check (Police clearance no less than 6 months old).
· Hold a current South Australian Driver’s License (minimum Provisional License).
· Be fully vaccinated for Covid-19 (minimum of two doses) - evidence will be required prior to appointment.
