About the Role:
We are seeking compassionate and dedicated Registered Nurses to join our Home Nursing Service team as a District Nurse. As a member of the Home Nursing Services team you have the unique opportunity to deliver services to a broad range of consumers and their carers in their own homes, clinic settings, local community centres and other suitable delegated areas. This enables staff to become involved in community education and health promotion, and to have input into the future directions of service provision in the region.
If you are registered nurse with recent experience in an acute setting, with the ability to work autonomously, undertake a varied and diverse workload predominately in a home-based setting and feel you can provide:
- General and specialised nursing and healthcare to consumers
- Improve consumers health status, whilst enabling them to enjoy the benefits of remaining at home and retaining independence.
- Care for patients in the Hospital in the Home program and Mary Coram @Home
- A willingness to increase your knowledge in advanced wound care assessment.
- Practical solutions to promote independence and safety at home and engage with multi-disciplinary teams to improve the outcome for HNS consumers
