Together, we can do moreThe Registered NursePalliative Care will work within the ISLHD Community Palliative Care Service to provide patient centred Palliative Care for people with advanced life limiting illnesses, their families, and carers. Working within a multidisciplinary team, the role provides direct care to clients and their caregivers in the home environment and works with primary care, community-based services, other specialist services or clinicians, aged care facilities and group homes.The role requires a commitment to Palliative Care and the ability to work independently in the community with guidance and support from senior Palliative Care clinicians. Responsibilities include Palliative Care assessment, symptom management and ongoing care planning and nursing care.This is a 7day/week service which operates from 6am- 1130pm every day (with patient contact between 6am and 11pm).
