- The successful applicant for this position will be located at Bulahdelah Health Service and be a part of the Generalist Care multi-disciplinary team.
- You will work among Community Health Nursing, a Nurse Led Clinic, Mental Health Services and other outreach services.
- The broader Community Health multi-disciplinary team includes Wound Management, Cardiac & Respiratory Rehabilitation, Continence, Dietetics, Diabetes Education, Wound Specialists, Immunisation and Audiometry.
- The Palliative Care team consists of Nursing Staff and Bereavement Counsellor; each small team is based to cover four areas across Lower Mid North Coast: Taree, Forster, Gloucester and Bulahdelah.
About the role:
- The successful applicant will be providing Palliative Care Nursing to patients in the community and surrounding areas.
- You will be carrying out home visits within the community providing Palliative Care nursing to patients.
- This position is supported by the Nurse Unit Manager and Palliative Care Clinical Nurse Consultant. For speciality tasks, the Wound Management Specialist and Clinical Nurse Educator will support you. You will also be required to attend annual clinical competencies in Syringe Drivers and other competencies relevant to the role. Additional education and training opportunities will be supported if they are appropriate to the core business of the Community Palliative Care Nursing team.
About you:
- We are searching for a highly skilled and experienced Registered Nurse with excellent communication and interpersonal skills, who applies evidence-based practice and critical thinking, with the ability to implement individualised person centred care.
- Our ideal applicant will be self-motivated, caring, compassionate and have post graduate studies in Palliative Care or willing to work towards same.
Requirements:
- Current registration as a Registered Nurse with AHPRA.
