About the Role
We're seeking a friendly & professional Registered Nurse with Primary healthcare experience to work on a unique 6 month project (with view to go permanent) which focuses on innovating changes to how customers interact with practices and creating in-practice behaviours to improve the provider and patient experience and ultimately keep people out of the hospital. We are running the pilot in our practices across Parramatta, Auburn and Campbelltown.
- Utilising the data systems (e.g. Best Practice, PenCS) to identify patients eligible for Case Review
- Coordinating and preparing necessary documents for the Case Review
- Getting patient consent
- Capturing and thoroughly documenting relevant information within the Best Practice system
- Completing administrative workup required for patient post Case Review (e.g. communicating any case review outcomes with the patient)
- Work in collaboration with a Social Worker to manage the prep, support and follow-ups of any case reviews.

What we are looking for
- A Registered Nurse who holds AHPRA registration with no restrictions
- Demonstrated strategic and reactive problem-solving skills
- Strong understanding of Chronic Disease Management
- Knowledge of recall/reminder systems, data and clinical software (PenCS and BestPractice)
- Extensive experience and knowledge of the general practice environment and Primary Care
- Outstanding interpersonal, team building, negotiation and communication skills including written, verbal and IT/IM
- Work independently and as a collaborative member of the multidisciplinary team
- Able to prioritise competing demands
