AddressHealth N Home specialises in Continence assessments via Telehealth and In person. We offer a comprehensive range of services to address your health and continence needs. These services include continence assessments, treatment and management options, and the supply of continence products and general community nursing.
We are looking for two Registered Nurses who are passionate about providing community nursing.
Continence Assessment and Reports
- Meeting appointment times Face to Face /Telehealth assessment/reviews
- Writing reports and continence prescriptions
- Requesting Product Quotes from suppliers
- Developing care plans
- Developing rapport with participants
Support Worker Education
- Attend face to face appointments in the client’s home or other offered facility for training needs
- Coordinate care with a multidisciplinary team as required
- Improving health outcomes – escalate need for further intervention when needed
Provide Catheter Services
- Indwelling and suprapubic catheter changes
- Routine catheter maintenance
- Develop care plans and provide review assessments of individual care plans
Complex Bowel Care training and support
- Complete assessment
- Complete report
- Provide prescription and advice
- Provide education for support workers and carers
- Develop care plan and provide review assessments of care plan
Wound Care
- Wound Assessments - simple and complex
- Wound Care plans, reviews and assessment of care plan based on outcomes.
- Supervision of support staff Client Care and Administration
- Maintain accurate and timely client clinical notes in practice tools in SharePoint management systems to ensure best care for clients
Diabetes Care Training, include insulin administration
- Carry out diabetes assessments on clients
- Maintain accurate and timely client clinical notes in practice tools in SharePoint management systems to ensure best care for clients
Requirements for the position:
- Bachelor of Nursing degree
- Current AHPRA Nursing Registration
- Complete an NDIS Worker Screening Check and working with children check
- Valid police clearance check
- Professional Indemnity to $10m
- Public Liability Insurance to $10m
- Reliable registered and insured vehicle
- Annual CPR certificate and First Aid Certificate
- Annual Immunisation Certificate (including COVID immunisation history)
