Our health centres service nine communities spread over 45,000 square kilometres in the Big Rivers region. The Sunrise footprint covers Aboriginal communities that have some of the most untouched beautiful landscapes to explore where the locals are welcoming and ready to share their culture.

How You’ll Make a Difference

Sunrise Health Service is looking for a full-time Rheumatic Heart Disease Coordinator based in Katherine, Northern Territory.

This role entails providing visiting services to Sunrise clinics and communities, focusing on the prevention, screening, diagnosis, treatment, and care of patients at risk of Acute Rheumatic Fever (ARF) and with Rheumatic Heart Disease (RHD). The position reports directly to the Integrated Health Manager and is responsible for the line management of Environmental Health Community Officers.

More specifically, your responsibilities include but are not limited to:

  • Providing prevention, screening, early diagnosis, assessment, treatment, and supportive care for patients at risk of and with ARF and RHD
  • Working collaboratively with all health center and specialist staff for appropriate planning and implementation of care
  • Conducting health promotion activities related to ARF, RHD, and increasing uptake of secondary prophylaxis injections
  • Overseeing referrals, recalls, and care for patients affected by ARF and RHD
  • Providing culturally safe and supportive referral pathways across healthcare levels
  • Maintaining processes for effective data collection on ARF and RHD for monitoring and evaluation
  • Supporting community workers in planning and reporting their environmental health activities
  • Ensuring resources are available for patients and families on prevention of ARF/RHD
What You’ll Bring to the Team

To be considered for this role, you will have a minimum of 2+ years of experience in ARF/RHD management and chronic disease care. You will understand and adhere to the principles of Aboriginal community control health services, ensuring sensitivity and cultural safety in remote Aboriginal communities. 

You will have:

  • AHPRA Registration
  • Advanced interpersonal and communication skills, both written and verbal
  • High-level analytical thinking and problem-solving
  • Demonstrable experience in quality improvement and understanding its relationship to strategic development
  • You will be committed to professional development and continuous improvement

Please note: A current open Driver's License is a requirement for this position. Confirmation of employment is dependent on the outcome of a Criminal History Check and successful application for an Ochre Card.

About the Big Rivers Region

The Big Rivers region (a 3 hour drive from Darwin) begins south of Kakadu National Park encompassing Pine Creek, then extends over 500km south to Kalkaringi and stretches across the width of the Territory between the Western Australia and Queensland borders. The region is significant in scale, covering over 360,000 square kilometres and accounting for an area greater than that covered by Germany. The region consists of a number of varied natural environments including vast rivers, tropical forests, rocky escarpments and savannah plains.
