We are a disability nursing company looking for a Senior Registered Nurse with a minimum of 5 years experience, to work as an RN Disability Service Coordinator, to join our friendly team in Blaxland. The role involves liaising with case managers, medical teams, families and the clients themselves.
We specialise in providing support for acquired brain injured clients and are looking for a Service Coordinator with industry relevant qualifications or demonstrable experience. Understanding of and experience in brain injury, spinal, bowel and catheter care, quadriplegic and disability support is preferred.
Key Responsibilities:
- Regular evaluation of client’s services
- Drafting of client Individual Support Plans
- Assistance with rotating roster of after-hours phone service
- Liaising with Case Managers and other professionals involved in a client’s care
- Day to day management of client’s needs
- Compliance with WH&S legislation and company policy and procedure
- Support with clinical care and clinical visits for clients

Essential criteria:
- AHPRA Registration
- Minimum 5years clinical experience
