The Opportunity
We are looking for a Senior Care Manager to join our supportive and professional multidisciplinary teams in a full time capacity.

Responsibilities include but are not limited to;

  • The management of a reduced client case load
  • Supervise and coordinate orientation of new staff
  • complete joint home visits with Care Managers as required and appropriate
  • support the HCP team to be responsible for working in partnership with the client and/or carer in receipt of Home Care Package
  • Supporting the HCP team to manage complex client issues and complaints in accordance with Organisational Complaints Policy and provide feedback to consumer/carer and the Manager.
  • Support the HCP team to develop and document consumer directed care plans, risk identification and management, conduct regular reviews of their needs and amend care plans as appropriate.
  • Liaise and advocate with other relevant service providers regarding individual care needs.
  • Regular reviews of client care plans, care notes and client file audits with the HCP team
  • Represent the organisation in both internal and external network meetings, marketing campaigns and industry wide network opportunities
  • Work with internal and external stake holders to promote the HCP team and internal and external service referrals

Skills & Experience

  • Minimum of 3 years’ experience in Home Care Packages program 
  • Tertiary qualification in Social Work, Allied Health, Nursing, Care Management or equivalent
  • Extensive experience within the Home Care Package Program 
  • Experience providing mentorship/leadership
  • A comprehensive working knowledge of the Home Care Packages Operational Manual, the Aged Care Act and an understanding of the Victorian Health Records Act.co
  • Demonstrated experience in Consumer Directed Care (CDC) choices and preferences to consumers
  • Knowledge of aged care related specialist services, i.e. Parkinson’s and Dementia support Services
  • Understanding of the interface between CHSP and HCP.
  • Advanced planning, organisational and time management skills
  • Proven ability to establish effective and cooperative internal and external working relationships
  • Experience in assessment, care management and service delivery to aged consumers living in the community
