A bit about the role
The Senior Case Manager Advocate, part of a close-knit team of 5 within the Access and Navigation Team of the Connected Communities Department, provides case management and advocacy support to individuals in our local community.
This role involves linking individuals, their families, and carers to support services, particularly those experiencing declining health and significant barriers to accessing the necessary support for maintaining their wellbeing. It's a fast-paced and challenging position, offering a rewarding opportunity to advocate for the local community.
What you’ll deliver
- Responsible for working with older people who are living at home with a clinical condition and/or specialised need not supported by a Home Care Package.
- Using a person-centred approach, draw on client strengths and resources to develop individual care plans and goals, encouraging the client their carer or families to participate in all stages of referral pathways and service provision, advocating when necessary.
- Provide outreach support by assisting and facilitating client's connections to services as required.
- Support immediate interventions for at risk clients, including VCAT guardian and administrator applications, and crisis housing advocacy.
- Build and develop strong working relationships with key internal and external stakeholders, to promote the program and assist with referral pathways.
- Undertake regular client reviews to determine service needs and monitoring of progress towards achieving goals.
What you’ll bring
- Tertiary qualification in Mental Health, Community Services, Social Work, Allied Health/Health or relevant other qualifications with substantial experience working as a support worker/Case Manager/co-ordinator of services in an outreach capacity.
- Experience with facilitating care co-ordination.
- Highly developed interpersonal and communication skills with experience in developing and maintaining relationships.
- Knowledge of the aged care/community services sector.
- Have experience using a strength-based approach to support people/using trauma informed therapy.
- Demonstrated experience in working with and understanding people with complex needs using a person centred strength-based approach.
- Willingness to undergo pre-employment background checks including National Criminal History Check, Working with Children Check and Eligibility to Work check.
- A curious, collaborative, and inclusive community mindset.
