About the Role
- Staff are expected to be compliant with the timely and accurate input and collection of consumer related demographic information, diagnosis, outcomes collections and provision of service activity into appropriate applications. The data and information includes the electronic entry and completion of documentation to meet legislative requirements, including Mental Health Act 2016 documentation, admissions, discharges and transfers.
- Develop and strengthen relationships with key partner agencies to enable effective and collaborative practice between Evolve Therapeutic Services and our stakeholders, that meet the cultural and mental health needs of Aboriginal and Torres Strait Islander children and young people.
- Participate in the state-wide Evolve Aboriginal and Torres Strait Islander Program Coordinator working groups as required to promote the ongoing development and implementation of an evidence-based model of service delivery which meets the therapeutic needs of Aboriginal and Torres Strait Islander children and young people in out of home care.
