Are you a passionate and experienced Senior Occupational Therapist interested in Emergency Department outreach?

We currently have Part-Time opportunities for Senior Occupational Therapists to work in an innovative program called CCT2Home. This Occupational Therapist is a crucial member of the trans-disciplinary Allied Health team called Care Coordination Team (CCT). The team’s primary role is assessing, treating and planning for a coordinated discharge for people to go home safely from the Emergency Department. For many of these people further allied health interventions are required after their assessment in ED to set them up to be independent at home. Usually, patients would be admitted for further thorough assessment and discharge planning to take place, however at Sir Charles Gairdner Hospital the CCT2Home Occupational Therapist continues the assessment and coordination of care in the patient’s own environment within the next day or 2 following discharge.

This is a Home Visiting role with a twist. You will provide rapid follow up to patients discharged from ED to provide falls, pressure, cognition and home environment interventions that will prevent a readmission. Home visiting skills are a bonus however knowledge of acute medical conditions and the impact on function and how to problem solve on your feet is more important!

This role is well supported by a P3 Coordinator and leadership team plus Senior Home Visiting teammates with exceptional knowledge and experience in complex discharge planning, community resources, equipment and home modifications. You will be in a team where you continually learn with plenty of opportunities for education in acute hospital care and a wide range of specialist clinical areas (e.g. domiciliary care, aged care, cancer, neurosciences, general medical/ surgical). With supportive supervision and career pathways in clinical, research and leadership streams, we are committed to helping our Occupational Therapists achieve their career goals.

To be eligible for a fixed term appointment you must have documentary evidence of your entitlement to live and work in Australia for the period of the contract.

This is a Recruitment Pool for Permanent and Fixed Term Part-Time appointments at various hours. Fixed Term appointments have the possibility of extension(s) and / or permanency.
