You will see patients in the community to support their early discharge from the Emergency Department or the acute wards; Hospital in the Home (HiTH) patients; or where intervention may prevent an acute admission. Patient groups may include functional decline post illness, post falls, post orthopaedic surgery, respiratory, and supporting cardiac and pulmonary rehabilitation.
