SalarySenior Social Worker – Grade 3
Sub-acute inpatients
- Provide expert clinical skills, leadership and knowledge in the sub-acute inpatient services including NDIS, Victorian Civil and Administrative Tribunal (VCAT) hearings, and My Aged Care.

- Geriatric Evaluation and Management (two roles)

- Provide exemplary social work service in the relevant sub-acute wards (GEM and/or inpatient rehabilitation) including assessment, intervention, and discharge planning.
- Foster positive team and workplace culture as a senior Social Worker.
- Utilise specialised skills and knowledge in relation to relevant community providers communicating between patients and caregivers.
- Demonstrate excellent communication strategies with patients, relatives, staff and relevant services in order to provide appropriate services, information and leadership
- Actively lead and participate in Allied Health quality improvement initiatives and research
- You have demonstrated hospital experience with a collaborative leadership style
- You have excellent communication skills and a passion for continued professional development for yourself and others.
- You are a qualified Social Worker, eligible for membership with the Australian Association of Social Workers.
- You value working with supportive and dedicated health professionals and multidisciplinary teams.
