Your opportunity
- Deliver high level Speech Pathology clinical services and leadership for consumers of Community Health and Therapy Services (CHATS), including outreach to regional areas, as determined through the Hospital and Health Services Plan and priorities.
- Provide organisational and clinical leadership to the CHATS in the Mackay Hospital and Health Service (MHHS). This incorporates clinical supervision, consulting, organisational and quality activities to assist with the ongoing service improvement.
- Work as a member of a transdisciplinary team, delivering high level clinical services to consumers in the Mackay Hospital and Health Services area, covering a range of clinical conditions and complexity. This position operates within a Professional Skill-Sharing and Delegation (PSSD) model of care by providing and participating in supervision and training in the use of skill-shared and delegated clinical tasks, and by delivering an expanded scope of clinical practice.
About the Team
Community Health and Therapy Services (CHATS) plays a vital role in the continuum of care for consumers and their carers at discharge from acute care through to re-establishment in the consumers' residence, or when necessary, transitioning to residential care settings. CHATS comprise a range of outpatient healthcare activities focussed on assessing, re-abling and maintaining individuals' capacity to successfully remain in their community.
The target consumer population is adults, including frail aged, and the carers of these consumers. Services encompass specialised and intensive rehabilitation, low-intensity rehabilitation, maintenance, and basic support, and are delivered via a trans-disciplinary team representing medical, nursing, occupational therapy, physiotherapy, social work, psychology, speech pathology, dietetics, specialist continence, allied health assistants and personal care workers.
Mackay Hospital and Health Service has six hospitals, two multi-purpose health services and four community health centres that employ more than 3,700 staff. The district covers an area of approximately 90,000km2 and provides services to patients and their families in Bowen, Cannonvale, Clermont, Collinsville, Dysart, Glenden, Mackay, Middlemount, Moranbah, Proserpine and Sarina.
