The position is 24 hours per week to be worked across 3 days
This position will be primarily based in Mortlake
We are currently seeking a suitably qualified or experienced Service Provision Officer to join our dynamic community care team. The successful applicant will be responsible to provide supervision and manage the rosters of assigned community care workers. Duties will also include the provision of quality, consistent and responsive in-home support services to clients within the community, funded through either CHSP, HACC-PYP, NDIS, HCP or our brokered service providers, in accordance with the relevant program policy and procedures.
QualificationsEssential
- Tertiary qualification in either disability, social sciences, health, community services or a related field
- Current drivers licence
Essential
- Experience in working with clients in health or community services, including review and evaluation of needs.
- Knowledge of client data base systems in particular Carelink.
- Demonstrated high level of communication skills both verbal and written with a wide variety of people.
- Demonstrated knowledge of the HACC PYP, Commonwealth Home Support Program, NDIS and related community care, disability support programs.
- Demonstrated ability to build productive relationships with a range of key internal and external stake holders.
- Ability to use initiative to problem solve and make informed decisions when required.
