About Us
The Institute for Urban Indigenous Health (IUIH) was established by its founding members to provide a coordinated and integrated approach to the planning, development, and delivery of comprehensive primary Health Care services for Aboriginal and Torres Strait Islander people within South East Queensland. 

We have an exciting new opportunity for a Social Health Care Coordinator to be part of our new Palliative Care Team, Quality and Innovation. This is a unique opportunity to be part of the team that provides leadership in the development, implementation, and evaluation of plans, programs, and models for improving Aboriginal and Torres Strait Islander health and wellbeing. 

About the Role
We are seeking a dedicated Palliative CareSocial Health Care Coordinator to play a crucial role within the Palliative Care Team to guide in the implementation of a new service model designed to meet the unique needs of Aboriginal and Torres Strait Islander individuals and their families. 

As part of multi-disciplinary team, the position ensures Palliative Care and End of Life Social Health services, supports and information are delivered in accordance with the IUIH Model of Care. The Palliative CareSocial Health Care Coordinator will support the Project Team and Member Services to establish and support connections to Social Health services and ensure that our patients and their families get the right supports at the right time.

With a focus on quality improvement, the role supports IUIH Member Services in their primary health clinics, as well as providing support for outreach services, across South East Queensland.  

Your day-to-day Duties may include:
As a member of the Palliative Care Team, the role has shared accountability for the design and delivery of the new Palliative Care service, including:

  • Collaboration with the project team and Member Services to identify ways to connect to existing Social Health programs and services across South East Queensland to develop a strength focused plan that sets out priorities and actions, including defining support/interventions from other Social Health providers and network clinics and services.
  • Supporting the mapping and scoping of the existing workforce skills, capacity, and training needs and options.
  • Developing responsive Social HealthPalliative Care, end-of-life care and voluntary assisted dying tools, training, resources, and information to support one-to-one discussions with clients, their families and with IUIH and Member Services.
  • Providing leadership in the design and development of Palliative CareSocial Health services, with responsibilities and oversight of safety and quality of care.
  • Providing Social Health services within the context of Palliative Care in a professional, confidential, and culturally safe manner.
  • Undertaking comprehensive assessment of clients’ psychosocial needs in consultation with the client and work with the client and their family to develop a strength focused plan that sets out priorities and actions, including defining support/interventions from other Social Health providers and network clinics and services.
  • Managing a caseload of complex clients ensuring systematic contact and follow up with clients to ensure priorities and actions are met, in conjunction with other team members, network providers (e.g., psychologists, counsellors, team leaders) to enhance levels of support and case support planning.

About You?

  • Current C Class Drivers Licence (QLD)
  • Current Criminal History Check
  • Current Blue Card
  • Current CPR Certificate
  • Minimum Diploma of Health Services / Social Service / Social Sciences or relevant qualification
  • Ability to work sensitively and effectively with Aboriginal and Torres Strait Islander communities, their leaders and health professionals, while respecting traditional culture, values, and ways of doing business.
  • Demonstrated experience supporting training and development initiatives and resources, with the focus on improving knowledge within a workforce and community.
  • Minimum three years’ relevant experience OR Case management experience in a comparable role.
  • High level communication skills that demonstrate knowledge, clinical reasoning, and the ability to effectively liaise with clients and members of community, team members, health professionals and clinic staff in the areas of death and dying, grief and loss and other psychosocial issues.

Desirable, but not mandatory:

  •  Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
