• Salary $86,950 - $100,729 p.a. (pro rata) AHP2 | Full Time, Ongoing

This social work position is based at the Repat Health Precinct, and involves working within Community GEM, a case management stream of the service specialising in care of high-risk older people with age related health issues such as cognitive decline.

About the Role:

As the Social Worker, your duties may include:

• Short Term Case management
• Independently assessing clients’ abilities, problems and needs using a variety of interdisciplinary assessment tools.
• Planning and implementing evidence-based intervention programs
• Implementing outcome measures appropriate to the client population to evaluate efficacy of therapy programmes.
• Arranging and supporting Geriatrician assessments
• Initiation, support and navigation of the SACAT process from application to outcome
• Prescribing, arranging and linking with appropriate care providers
• Communicating with clients’ family, carers, and other service providers

To be successful in this role, you will need: 

• An appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Worker
• Demonstrated experience in clinical management within geriatric services, including program evaluation and service development, combined with a broad base of experience in social work practice
• High level competency and extensive knowledge in the application of a wide range of social work assessment and therapeutic interventions as a member of an interdisciplinary team to clients with complex care needs.
• Ability to contribute to professional education, the training of discipline specific students and to research.
