Social Worker Grade 1
Full Time Fixed Term (until 15/3/2026)
80 hours per fortnight with monthly ADO
About the Role
The Social Work team provides patients with additional support to improve psychological and social functioning and we ensure each patients social, emotional and cultural needs are met. We minimise the negative impacts of illness and hospitalisation on patients. We are a dynamic and innovative group of 30 plus Social Workers who are always willing to share knowledge and peer support.
There are regular department in-services, Grade 1 individual and peer group supervision, external professional development opportunities, you are able to access study leave for higher education. There are many opportunities to develop your career and knowledge
Key Responsibilities
This position is responsible for Social Work services in Subacute (Aged Care and Rehabilitation) and Acute Medical wards, in Ballarat, but will also require flexibility into other clinical areas. You should be skilled in comprehensive psychosocial assessments, and an interest in working in health and multidisciplinary teams. You will be aiming to make a difference to patients, families and teams. As the successful applicant you will have experience and competency in grief and bereavement support, care planning, decision making, liaison with internal and external stakeholders.

Conducting comprehensive clinical assessments
Ability to work within a fast-paced environment whilst maintaining a professional manner
Ability to plan and problem solve
Demonstrate sound knowledge in Social Work theory and practice skills
Working knowledge of Acute/Subacute hospital systems and resources
