This integral role encourages best practice philosophy to provide high quality clinical care to patients with a broad range of complex medical and psychological needs, demonstrating an advanced skill set. Emcompassing a clinical caseload within the Acute and Community streams, including Colac Area Health's Palliative Care Service, you will form part of a supportive and collaborative multi-disciplinary team and will be required to provide highly responsive and flexible services to consumers.What’s required?
- Bachelor of Social Work degree and eligible for membership with the Australian Association of Social Work.
- Proficient clinical skills in undertaking comprehensive psychosocial assessments, clinical reasoning, and formulating appropriately targeted treatment plans/interventions with achievable goals for clients in a variety of settings.
- Provide care coordination, brief intervention counselling, advocacy, mediation, referral and support for individuals, families and groups in the inpatient, outpatient and community setting.
- Demonstrate clinical practice experience in acute care, rehabilitation, palliative care, aged care, community health and outreach services with an awareness and understanding of community resources and support systems.
- Ability to work both autonomously and within multidisciplinary teams to achieve desired patient outcomes with a commitment to person centred practice.
- Prioritisation of caseload and management of competing priorities including delegation where appropriate.
- Experience in providing supervision to Grade 1 Social Workers and students to promote professional competence and growth in staff.
