Work as part of an interdisciplinary team providing best practice social work services and advice with Timboon and District Healthcare Service (TDHS). Deliver a range of services within the Social Worker/Counsellor scope including psycho-social assessment, discharge planning, counselling, education, referral, consultation and liaise with patients, clients, family members, staff and management of TDHS.
Duties include:
- Provide best practice social work services within interdisciplinary team.
- Conduct psycho-social assessments and discharge planning.
- Offer counseling, education, and referral services.
- Liaise with patients, clients, family, staff, and management.
- Collaborate on clinical management and develop group therapy programs.
- Promote and participate in quality improvement activities and initiatives.
This position is fixed term part time 16 hours per fortnight. The classification will be Grade 2. Terms and Conditions of Employment will be in line with the Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021-2026.
Qualifications, Skills and Experience
To be successful in this role you will have:
- Bachelor of Social Work or appropriate relevant tertiary qualifications
- Eligibility of or Registration with the Australian Association of Social Work (AASW)
- Experience in a hospital, community health service or other equivalent health field
- Proficient clinical skills in psychosocial assessment including risk assessment, complex care planning, discharge planning, Social Work interventions and communication
- Knowledge and understanding of the Social Work role in acute care, rehabilitation, palliative care, aged care and community health services
