About Us
SECU provides contemporary, high-quality, interdisciplinary secure extended care and treatment services for consumers with chronic psychiatric conditions and/or complex multi-diagnostic presentations. A collaborative multi-disciplinary team provides targeted treatment and rehabilitation programs that maximise individual functioning, minimise the barriers to recovery, and promote return to community living where possible. This role offers an opportunity for the incumbent to join the unit’s highly skilled team. It is a fast paced, demanding and rewarding role and will require the incumbent to demonstrate a variety of mental health skills and abilities.
About the Role
The Grade 1 social work role is integral in assisting our consumers to improve life skills, access housing, develop a better understanding of their financial matters and link with different services and support, such as Centrelink, National Disability Insurance Scheme, employment and legal matters.
To support the above, the Social Worker is expected to provide individual assessment, support in developing interventions based on findings from the assessments, contribute to the overall treatment regime and equally contribute to the coordination of discharge planning for example:
- Provide information on supporting services and where applicable, coordinate referrals, application and communication with other services including:
- NDIS (National Disability Insurance Scheme)
- Centrelink
- Support workers
- Forensic services
- Spiritual Care
- Legal Aid
- Drug and Alcohol Counselling
- Regular communication with families and carers
- Lead group activities to facilitate social interactions
There is a need to be collaborative in practice as there is an expectation of involving and engaging with the consumer, their family and carers, the treating team and the wider stakeholders who are involved in the care of our consumers. It is a role that is under pinned by Monash Health iCare values and vision statement. The incumbent will be required to work in any clinical area at any or all campuses of Monash Health as and when
required. Such a requirement may be a temporary arrangement, on an ad hoc basis, or a permanent change.
