BACKGROUND
UQ HealthCare Ltd (UQHC) is a not-for-profit company owned by the University of Queensland. UQHC operates multidisciplinary primary health care centers with teams of experienced GPs, nurses, allied health practitioners and specialists dedicated to providing compassionate care and service, and the highest quality, integrated, multi-disciplinary healthcare.
UQ HealthCare , Health and Wellbeing Queensland (H&WQ) The University of Queensland (UQ) and Griffith University (GU), have formed an alliance to develop Logan Healthy Living (LHL), a model of integrated chronic disease care, which leverages the expertise and resources of a range of partners, recognizing that successful prevention and management of chronic disease requires a collaborative approach.
Logan Healthy Living was established in April 2022 to pilot a model of integrated care that connects care for complex chronic disease and empowers patient self-management. Logan Healthy Living delivers Lifestyle Management Programs for Type 2 Diabetes Mellitus (T2DM).
An interprofessional approach to address the physical, mental, social and economic determinants of chronic disease across the spectrum is required.
Through existing partnerships with UQ and Griffith, UQ HealthCare has employed an interprofessional team of allied health professionals including the deployment of a student-infused allied health workforce in the LHL environment.
LHL is being delivered from our purpose-built facility at Meadowbrook (completed Feb 2023) which is collocated with our Meadowbrook Medical Centre and the Logan Endocrinology and Diabetes Services (LEADS) – delivered by Metro South Hospital and Health Service (MSHHS).
The LHL Lifestyle Management program works as follows:
- Initial patient assessment, 8 group sessions (education & exercise), plus 1-on-1 consults, supported by technology.
- Exercise and diet plans; psychology to assist behaviour modification; pharmacist medication management, and social work to address socioeconomic barriers.
- Reassessments at 1,3,6,9 and 12 months (at no cost to the patient) to ensure continued motivation, connection to H&WQ and other programs for ongoing healthy lifestyle maintenance.
Our university partners have designed and implemented an evaluation framework to assess the effectiveness of these new approaches to patient care and leveraged the depth and breadth of research talent across both universities to further contribute to clinical service development, as well as provide access to additional revenue streams through grant funding.
DUTY STATEMENT
Social Prescribing Link Worker – Logan Healthy Living
Primary Purpose of Position
This newly created and innovative position will play a pivotal role in integrating people living with chronic health conditions, and others, into the local communities to help improve their health and wellbeing, through Social Inclusion. Working closely within the interprofessional allied health team at Logan Healthy Living, General Practitioners, Practice Managers and external organizations you will manage a case load of individuals working towards reducing social isolation & improving community connectedness by supporting participants to, as able, reduce their use of mainstream health services and enable people with chronic health issues to lead healthier lives and better manage their condition/s.
Duties and responsibilities include, but are not limited to:
Holistic Client Care
- Provide direct Social Prescribing services to participants within intake assessments, 1:1 consultations and group education sessions as required to facilitate positive outcomes.
- Connect people to their local community and services within it, and providing effective feedback in order to develop services appropriate to the needs of the community.
- To liaise with relevant organizations to access appropriate services required by participants to meet their short and long term goals
- Provide individual support to participants, including attending new activities with them
- Develop, monitor, and regularly review clinical guidelines, policies, and procedures in consultation with discipline expertise and consumers.
- Work with the Clinic Manager to contribute to Social Prescribing & Social Work components of effective Clinical Governance including clinical service reviews, clinical management software system reviews, risk management, clinical audit and review of patient feedback/surveys.
- Provide advice and assistance to ensure clinical practice materials are updated regularly to reflect current practice systems and processes.
Program/Service Development & Research
- Contribute to the design, delivery, and evaluation of appropriate clinical programs for implementation at UQHL.
- Contribute to the development of appropriate resources (to support new program delivery).
- Participate, if requested, in research projects related to clinical practice and evaluation of the lifestyle management program
Clinical/Student Education
- Provide direct supervision and clinical education for undergraduate and/or post graduate Social Work students
- Assist in the development and implementation of appropriate IPE student education processes and models of clinical supervision to support students' learning as appropriate for their existing knowledge and skill level
- Consult and communicate with clinical education and academic staff within and across professions
- Contribute to problem solving around IPE and profession specific clinical education issues;
- Provide clinically appropriate interprofessional education and supervision to individual students and groups of students;
- Monitor, evaluate and report on students' development of clinical skills and knowledge;
- Foster the development of inter-professional and profession specific education.
- Conduct quality assurance on students completing placements
- Conduct mandatory evaluations and provide feedback to students & universities when appropriate
- Ongoing knowledge sharing with broader team

Key Selection Criteria
Essential
- A Social Work degree with current status of an Accredited Social Worker or Accredited Mental Health Social Worker
- At least 2 yrs post-graduate experience.
- Current First Aid and CPR certificate.
- Eligibility for a Medicare Provider Number
- Current Driver’s License.
- Current QLD Paid Blue Card or ability to obtain one.
- Current National Police Check or ability to obtain one.
- Up to date immunization evidence upon request.
Desirable
- Experience in an interdisciplinary allied health environment with chronic disease populations
- Experience in Clinical Education of tertiary level students
