Tweed Community and Allied Health are seeking a Specialist Palliative CareSocial Worker to work as part of a multidisciplinary team providing inpatient and community social work services to clients, families and carers of the Specialist Palliative Care service. Working within the model of Palliative Care to identify and address psychosocial needs, provide therapeutic services to clients and provide specialist consultation and other team members in the management of complex cases. If you have: Relevant qualification in Bachelor of Social Work and eligible for membership of The Australian Social Work Association prior to commencing. To be appointed as a Level 3 Health professional evidence must be provided of three years' clinical experience and demonstrated experience of extensive specialist knowledge or a high level of broad generalist knowledge within the discipline as defined by the award definitions of a level 3 practitioner Extensive clinical experience in providing social work services, psychosocial assessment, case planning, individual and family grief and loss counselling and bereavement counselling Ability to utilise problem solving skills and a multidisciplinary team approach in the planning, delivery and coordination of patient care Understanding and knowledge of Aboriginal social, cultural and economic issues with capacity to flexibly develop local responses to community needs using cross cultural communication skills We can offer you: · Flexible and supportive work environment · Diverse career pathways · Professional and personal development · Opportunities to start or build a career, or re-enter the workforce · Challenging, inspiring and rewarding roles, and · The opportunity to make a difference!
