We have an exciting opportunity to work in a dynamic team leading our approach to the provision of quality care to clients of the Social Work Department This position will include the provision of comprehensive social work services to the patients, family and carers of Heart Lung Transplant Clinical Stream St Vincent’s Hospital Sydney and other clinical areas as allocated. This role will be working as part of a social work team/s and responding to clinical demands to maximise bio-psychosocial care and to enhance patient, family and carer communication with the treating teams. This position requires an ability to prioritise and organise a clinical workload as part of social work and multidisciplinary team/s.

Selection Criteria:
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia
2. Degree in Social Work and eligible for membership of the Australian Association of Social Workers (AASW) with evidence of clinical experience in a tertiary hospital or health related field as a practising Social Worker.
3. Demonstrated clinical skills and knowledge in psychosocial assessment, counselling, therapy interventions, case management, advocacy and discharge planning.
4. Demonstrated knowledge of crisis intervention, short term solution focused intervention and grief and loss.
5. Demonstrated experience and ability to work as a member of multidisciplinary teams in a high pressure environment with competing demands with sound, negotiation and liaison skills.
6. Well-developed time management, prioritisation, computer, written and verbal communication skills.
7. Ability to contribute to research projects, quality improvement and education activities within the designated specialties and social work services.
8. Ability to participate in an extended hours on call service.
