About the role:
Our Social Worker is considered a vital component of the SNF program, adept at functioning both independently and collaboratively.
In this position, you will exhibit your problem-solving abilities and implement a multidisciplinary team approach to plan, deliver, and coordinate patient care.
The Social Worker must build healthy therapeutic relationships, focusing on respect and empowerment for both clients and team members.
Where you’ll be working:
- Wallsend is a lively suburb of Newcastle close to a variety of shops and restaurants.
- The Sustaining NSW Families program is a structured home-based support and intervention program for children and families from pregnancy to 2 years of age.
- The successful candidate will be joining an enthusiastic multi-disciplinary team consisting of Nurses, Allied Health Clinicians, and Administration Support, all working toward supporting vulnerable families from pregnancy to a child’s age of 2 years.

- This role will be supported by the NUM Clinical Coordinator and be orientated to the role.
- In this role you will engage in group and individual clinical supervision. Peer social work support along with support from the Nurse Manager of Child and Family Health Service is available.
Requirements:
Our successful candidate will possess the following:
- Holds as a minimum a bachelor’s degree in social work which provides eligibility for membership of the Australian Association of Social Workers, or other qualification deemed equivalent by the employer.
- Eligibility to drive in NSW and willingness and ability to travel for work purposes.
About you:
- You are enthusiastic and have significant demonstrable experience working with children and families to enable them to work autonomously, utilise independent clinical reasoning, attend psychosocial assessments, and advocate on behalf of the families.
- You will need to demonstrate a proven ability to work effectively as part of a team.
