Role:
Based in ACT, your responsibilities will include:
- Providing comprehensive services to inpatients (including inpatient rehab, orthopaedics, general surgery, cardiac and neurosurgery, and ICU/CCU patients).
- Delivering excellent clinical services related to assessment, treatment, planning and evidence based practice
- Participating in program development and quality improvement activities within the Department
- Working closely with the multidisciplinary team to provide comprehensive care to patients and their support people
- Discharge planning with the multidisciplinary team and referrals to external services.
Selection Criteria:
- Tertiary qualifications in Social Work, with eligibility for AASW membership.
- Demonstrated experience working in a hospital setting.
- Experience and skills to conduct client assessments, and develop care plans that support clients’ needs.
- Excellent organisation, time management and self-management skills
- Highly developed communication skills and interpersonal skills
- Commitment to ongoing education and the provision of evidence based practice
