Responsibilities:
- Assess the psychosocial needs of patients and their families.
- Develop care plans in collaboration with the healthcare team.
- Provide counseling and emotional support to patients and their families.
- Assist with discharge planning and coordination of post-discharge care.
- Connect patients with relevant community resources and support services.
- Advocate for the rights and well-being of patients.
- Conduct family meetings and facilitate communication between patients, their families, and healthcare professionals.
- Maintain accurate and up-to-date documentation and records.
- Participate in interdisciplinary team meetings and case conferences.
- Stay updated on best practices and trends in social work and healthcare.
Requirements
- Bachelor's or Master's degree in Social Work or a related field.
- Professional License SCFHS is a must.
- Previous experience in a social work or healthcare setting is preferred.
- Strong knowledge of social work principles, ethical standards, and relevant laws and regulations.
- Excellent communication and interpersonal skills.
- Empathy, compassion, and the ability to establish rapport with individuals and groups.
- Ability to assess and evaluate psychosocial needs and develop effective care plans.
- Strong problem-solving and critical-thinking abilities.
- Ability to work effectively in a multidisciplinary team.
- Proficiency in using computer systems and electronic health record software.
- Fluency in English and Arabic.
- Ability to maintain confidentiality and handle sensitive information with professionalism.
