You will have significant experience with NDIS plan utilisation and funding arrangements. You will have equivalent to a VET Certificate IV in relevant studies and/or equivalent knowledge and experience within the disability industry. You have excellent supervision skills and are able to lead, coach and motivate a team. You will have the ability to work autonomously and within a team environment. You will have thorough working knowledge of all relevant government legislation and standards with the ability to understand and implement organisation policies and procedures. You will have a current WA ‘C/CA’ class driver’s license and capacity to travel as needed within the Greater Perth Metropolitan area. You will demonstrate a can-do attitude, like a challenge, are flexible, value people, choice, culture and diversity.
The job - What you’ll be doing
You will be responsible for design and delivery of the highest quality of professional care for people with high intensity support or complex requirements. You will supervise and support a Neighbourhood team, ensuring NDIS compliance. You will also implement, and manage customer support plans, proactively engaging and communicating with customers, their families, and stakeholders.
Your responsibilities
- Model Interchange values within Neighbourhood teams, empowering employees to make good decisions.
- Develop High Intensity Customer Support teams that meet the needs of our complex customers and work in partnership with customers and families providing quality and safe supports.
- Ensure customer plans, assessments, reporting and case notes are completed within appropriate timeframes.
- Adhere to reporting requirements, ensuring required reporting is completed for relevant stakeholders.
- Actively work in and remain knowledgeable on current legislation, government regulations and Quality and Safeguarding, ensuring services remain compliant.
- Contribute to continual improvement processes, including identifying current trends and potential improvements to service delivery.
- Provide support as an out of hours responder.

- This role is permanent, initially working 38 hours per week.
- Predominately home based, with capacity to travel as needed.
