The primary role of the Spinal Cord Injury (SCI) case manager is to lead, guide and manage projects to improve complex discharges within the Spinal Cord Injury Service of the Northern Sydney Local Health District (NSLHD) and implement high levels of clinical expertise in order to provide high quality care consistent with NSW Health and policies, procedures and NSLHD standards.

What you'll be doing
- To act as a link between the two service providers of the NSLHD spinal Injury Service (Royal North Shore Hospital and Royal Rehab).
- To coordinate the implementation and evaluation of the SCI rehabilitation clinical pathway.
- Facilitate and enable discharge of identified individual patients with particular barriers to discharge, through working collaboratively with all disciplines in ensuring the actions and time frames agreed to in the pathway are progressed.
- Establish working relationships with key external service providers and stakeholders to develop and strengthen communication channels, relationships, timely responses and greater understanding about the discharge needs of people with a SCI.
- Investigate and develop alternative rehabilitation and transition care options which can assist SCI patients in timely access of appropriate treatment, housing and care.
- Provide information and data to advocate and influence change.
- Develop policies, work practices and business rules to improve the effectiveness and efficiency of discharge planning.

Candidates will need to meet the following criteria:
- Degree / diploma as an allied health professional with current registration as accepted by AHPRA or relevant professional association. Allied Health professional requires extensive experience at a level 2 and nurses require a minimum of 5 years full time post grad experience and a relevant post graduate qualification.
- Highly developed verbal and written communication skills and capacity to influence at all levels including the ability to negotiate with community stakeholders to improve integration of multiagency services for complex patients
- Demonstrated knowledge of professional and clinical issues relating to Spinal Cord Injury across the health care spectrum
- Demonstrated knowledge and understanding of the machinations of key external agencies and current policy relevant to the ongoing care of SCI clients
