ABOUT THE ROLE
Support Coordinators are the crucial link between NDIS participants and quality services and supports.
As a Support Coordinator, you will:
- Onboard NDIS participants for ERC Support coordination service
- Develop and maintain effective collaborative relationships with NDIS participants, families, carers, advocates, and other service providers.
- Match NDIS participants with compatible services, supports, and programs according to their individual needs and goals.
- Monitor NDIS plan outcomes and expenditure in partnership with plan managers to ensure the plan remains in budget.
- Provide a personalized, empathetic, and responsive service and function as the main point of contact for your client base.
- Provide clear and accurate record keeping of case notes, information, and billing.
- Assist NDIS participants with the plan review process.
Qualifications & experience
- Minimum 6 months' experience in a Support Coordination role, LAC, NDIS Planner or similar are required, or strong evidence showing your knowledge and indirect experience around the Support Coordination role.
- Qualifications in Community Services, Allied Health, or a disability related discipline (or working towards one) is highly regarded but not essential.
- FAID and CPR Certifications.
- Excellent communication skills in written and verbal and ability to prepare clear and accurate reports and correspondence that are appropriate for the purpose and audience.
- Ability to plan and manage own workload in a complex environment.
- High level problem-solving abilities, as well as the ability to work effectively under pressure.
- Strong inter-personal skills and the ability to communicate effectively with a diverse range of people and organizations.
- A commitment to responsiveness and delivering a truly premium support coordination experience.
- A current Australian driver’s license
- Working with Children Check, Vulnerable Person Related Employment, NDIS Worker Orientation Module, Covid 19 Digital Vaccination Certificate or Immunization Statement.
