Brighter Futures Consulting are a locally owned and operated company based in Darwin, providing services to NDIS participants of the Northern Territory - including Coordination of Supports, Specialist Support Coordination, Specialist Behaviour Supports and Music Therapy.

We are searching for a pro-active and motivated Support Coordinator to join our team.

About the position:

The role of a Support Coordinator is to coordinate the supports for NDIS participants and assist them in achieving their goals outlined in their NDIS funded plan.

A Support Coordinators role is to proactively identify, develop, coordinate and facilitate access to supports and services alongside their allocated NDIS Clients. 

The position requires a highly organised person with excellent oral, written and interpersonal communication skills. Required to work well both independently and cooperatively as a member of a team, the position requires a person who displays self-discipline with the ability to avoid distractions and diversions in order to manage their own time effectively.

Key responsibilities: 

  • Work with participants, families and carers to identify and access opportunities for skills development, education and employment
  • Support and encourage participants to achieve their goals and provide support coordination services in line with their NDIS plans
  • Develop networks and collaborate with stakeholders to proactively identify, plan, implement and evaluate supports.
  • Prepare and maintain up to date and accurate Case Management notes and reports using clear and concise language on client progress.
  • Liaise with a wide range of stakeholders, community services, government agencies, customers, and service providers.
  • Attend and contribute to team meetings, discussions, and planning. Generate ideas and options, whilst maintaining flexibility and receptiveness to change and alternatives.

  • Working with Children (Ochre) Card
  • National Police Clearance
  • First Aid Certificate
  • NDIS Worker Screening
  • The right to work in Australia.
