We Care Support Services are seeking an experienced and agile Support Coordinator to join us on a long-term part-time basis, dividing your working days between two of our offices, providing support to our participants and building our presence within the newer of our locations.
The role offers 3 to 4 days per week - applicants seeking casual work are also welcome to apply!
Duties & responsibilities:
- Split your time working between our Biddaddaba and Redlands offices, as well as attending weekly community days
- Facilitate the rapid expansion of our newly established office in the Redlands by swiftly cultivating a robust clientele base,
- Foster strong community, mainstream, and disability provider relationships
- Provide support coordination, and/or psycho-social recovery coaching, and/or case management to NDIS participants with diverse support needs
- Develop and implement support plans with clients and monitor their progress and outcomes
- Identify and provide a wide range of solutions to complex challenges, barriers and risks that participants face, working collaboratively with stakeholders
- Demonstrate a strong solution focus on thinking outside of the box and creativity
- Contribute to organisational development and community networking
- Adapt to progressive changes to participant circumstances
- Provide a welcoming, inclusive, respectful support network for people from diverse backgrounds
Experience & qualifications:
- Minimum of 2 years’ experience in NDIS Support Coordination is essential
- Willingness to travel between office locations
- Qualifications in community services, disability, or other health related disciplines, minimum of Cert 4
- In-depth knowledge of the NDIS rules and operational guidelines
- Current QLD driver’s license & your own fully insured and safe vehicle
- NDIS worker screen clearance & blue card
- First Aid and CPR certifications
- Excellent interpersonal skills to foster productive working relationships with an array of stakeholders
- Permanent Australian working rights & current residence
- Computer literacy and proficiency in Microsoft suite
- Ability to work autonomously and take initiative
